Senior student / Parent “To Do” List

1. By April 23: RSVP to participate in Senior Celebration on Sunday, May 19
   1. Complete this RSVP form and make the $35 payment online (or follow instructions to mail check)
   2. Print the SuperGames permission slip waiver, sign (if under 18, parental signature needed) and either:
      1. Scan or take a photo of completed form & email to UASeniorCelebration@gmail.com with SUBJ: SUPERGAMES – LAST NAME, FIRST NAME, or
         1. Save the Scan as your child’s last name!
      2. Mail to Karin Cramer, 2480 Coventry Road, Upper Arlington, OH 43221
      3. Print and drop in the Senior Celebration mailbox in the Attendance Office.
   3. Pick up your T-Shirt during lunch periods on Monday, May 13 or Tuesday, May 14
2. By May 8: Purchase a graduation banner (new deadline!)
   YOU MAY STILL ORDER A BANNER, BUT YOU NOW NEED TO CONTACT SIGNMASTER DIRECTLY TO ARRANGE TO PICK UP YOUR BANNER AT THEIR FACILITY OR PAY TO HAVE IT DELIVERED DIRECTLY TO YOU. ORDERS PLACED MAY 9 AND LATER WILL NOT BE DELIVERED TO SCHOOL FOR THE MAY 14 PICK UP.
   
   1. Order your banner online
      (If you select Option B / Photo, Gradbanners will contact you about submitting your photo once your order is received.)
   2. Pick it up on Tuesday, May 14, 2:15-4pm in the UAHS Courtyard; delivery is available for an extra charge
3. For Parents Only:
   1. By April 23: Donate a Raffle Item or Cash to be awarded as prizes at Senior Celebration. Donations also may be used for scholarship  for this event.
      We are looking for:
      • Gift cards (gas, movies, food, bookstores, VISA gifts cards, etc.)
      • Small electronic items (smartphone/computer accessories)
      • Stuff for a dorm room and/or college life
      • Make check payable to “UAHS Class of 2019” and we will do the shopping for you
